In partnership with the Oregon Health Authority and Oregon Department of Human Services, the Oregon Health Plan Care Coordination (OHPCC) program strives to enhance the lives of everyone we help through improving their health and healthcare. With the OHPCC program, we focus on providing intelligent value to make a positive difference in the lives of the people we serve.

The OHPCC program is offered at no-cost to Oregon Health Plan qualified, fee-for-service Medicaid members who are not enrolled with a Coordinated Care Organization. We support you, our members, through medical care management programs and 24-hour Nurse Advice Line.
